Kepa went down injured in the final moments of extra-time in the Carabao Cup final with Manchester City, and Sarri readied sub-keeper Willy Caballero to come on.
Caballero was ready and waiting on the touchline, but Kepa insisted his fitness and defied his manager's orders to leave.
As the tie then moved into the penalty shootout stage, Kepa was seen winking into the broadcasters' camera - moments after refusing to leave the field.
